Instructions

• Click the Group History download button above to download a fillable PDF document.

• Save the document to your computer, open it and fill out the requested information for your group.

• After completing the form to your satisfaction enter your name and *email (*required) in the box to the right.

• Then click the upload button, navigate to your file and upload it.

• Lastly, click the "submit" button

Your form will be emailed to the Archive Committee.
